WHY PHYSICIANS NEED SPECIALIZED ACCOUNTING

Why Physicians & Doctors Need Accounting Support in Alberta & Canada

Physicians face their own financial challenges, including provincial billing, equipment financing, uneven income and regulatory requirements. Working with accountants who understand medical practices helps keep your practice profitable and compliant.

DOES THIS SOUND FAMILIAR?

Is Managing Your Practice's Finances Keeping You Up At Night?

Many physicians struggle with financial management on top of patient care. Here are three common situations:

Paying Top Marginal Tax Rates

You’re incorporated but unsure if your structure is optimized. You may be paying tax at Alberta’s top combined personal rate of 48% on income above $370,220. A proper corporate and holding company structure may let you keep more after tax, depending on your situation.

Incorrect Bookkeeping & Tax Planning

Years of inaccurate bookkeeping or tax planning can quietly cost you in missed deductions, penalties and interest.

Clinic Treated as "Just a Practice"

Your clinic may not be treated as a scalable, transferable asset. Without proper structuring, patient payment systems, and exit planning, you may be leaving unrealized value on the table.

PLANNING AND STRUCTURE

How We Help Physicians Build Wealth

Four of the ways we work with incorporated physicians across Alberta and Canada.

Individual Pension Plan (IPP)

We can set up an Individual Pension Plan sponsored by your corporation. Depending on age and salary, an IPP can allow higher contributions than a standard RRSP.

Holding & Investment Company Structures

We plan holding and investment company structures that can keep investment assets separate from your practice and support tax-efficient movement of funds between your companies.

Corporate & Personal Restructure

We review past bookkeeping and tax filings, correct what is wrong, and restructure where it helps.

Aligning Personal & Corporate Plans

We help you align salary, dividends, holding structures and retirement planning so your personal and corporate plans work together.

How do Alberta physicians save tax with a professional corporation?

Most incorporated physicians in Alberta save tax by keeping the income they do not need for living costs inside their medical professional corporation. If the corporation qualifies for the small business deduction, its active business income up to $500,000 a year is taxed at 11% in Alberta: 9% federal (CRA, corporation tax rates) plus 2% provincial (Government of Alberta, corporate income tax). You pay personal tax only on what you take out as salary or dividends, so the saving is a deferral, not tax-free income, and the money left in the corporation can pay for equipment, a clinic or investments.

To incorporate, you need an active registration with the College of Physicians & Surgeons of Alberta, which issues the professional corporation permit (CPSA, incorporate my practice). We keep your corporation’s books, file its T2 return and help you plan the salary and dividend mix each year, from our Calgary office for physicians across Alberta. The Medical Professional Corporation package and Clinic Accounting are fixed monthly fees, listed on our pricing page. Book a free consultation.

Should an Alberta doctor take salary or dividends from a medical professional corporation?

Most incorporated doctors use a mix of salary and dividends, and the right mix can change from year to year. The main differences, from the Canada Revenue Agency and CPSA:

  • Salary is employment income. It counts as earned income for RRSP room, which is 18% of the previous year’s earned income up to the yearly limit (CRA, RRSPs and other registered plans). Canada Pension Plan contributions are paid on it (Canada.ca, CPP contributions).
  • Dividends are not earned income, so they do not create RRSP room, and no CPP contributions are paid on them.
  • Family shareholders. The spouse, common-law partner and children of a physician with active CPSA registration may be non-voting shareholders (CPSA, who can be a shareholder). Dividends paid to family members can be taxed as split income at the top rate unless an exclusion applies (CRA, tax on split income).

Each year we look at your corporation’s books and your personal plans, work out the salary and dividend mix with you, and set your tax instalments to match. Book a free consultation.

How is AHS or clinic income taxed, and do doctors charge GST?

Employee or self-employed. When Alberta Health Services, a hospital or a clinic pays you, the tax treatment depends on the whole working relationship, not only on what the contract calls you. The CRA looks at things like who controls the work, who provides the tools and equipment, and who carries the financial risk (CRA, employee or self-employed). Bring your AHS, hospital and clinic contracts to the consultation and we will check how each one should be reported.

GST/HST. Most health, medical and dental services performed by licensed physicians or dentists for medical reasons are exempt, so you do not charge GST on them. On exempt services you generally cannot claim input tax credits for the GST you pay on related costs (CRA, type of supply). Services that are not for medical reasons can be taxable, so we check what you bill.

Should a physician choose a national accounting firm or a smaller CPA firm?

It depends on how you want to work. 786 Venture CPA is a CPA-led firm in Calgary, licensed by CPA Alberta. Every engagement is led by a Chartered Professional Accountant, and the same team handles your corporation’s bookkeeping, its T2 return, your personal return and your planning, so nothing is passed between departments. We serve physicians across Alberta, and in every province and territory except Saskatchewan, by Zoom, phone and email from our Calgary office. Our fees are fixed and shown on this page, so you know the cost before you start.

A national firm can suit a large clinic group that wants offices in several cities. If you are a physician with a professional corporation, or a clinic owner who wants one team that knows your whole picture, book a free consultation and compare.

What tax deductions do Alberta physicians often miss?

Most missed deductions are ordinary costs that never reach the return because the record was not kept. What you can claim depends on how you are paid: as an employee you claim less than a self-employed physician or a medical professional corporation, so we first check how each contract should be reported. This is the Canadian checklist we go through with every physician:

  • CPSA and professional dues. In a practice, annual licence fees and dues to keep a membership are deductible business costs (CRA, Business expenses, page dated 2026-08-31). As an employee, professional board dues required under provincial law and dues to keep a professional status recognized by law go on line 21200 (CRA, Line 21200, page dated 2026-01-20).
  • CMPA and liability protection. On an employee return, line 21200 includes professional or malpractice liability insurance premiums (CRA, Line 21200, 2026-01-20). In a practice or a professional corporation, it is a cost of earning the practice income; keep the yearly statement (CRA, Business expenses, 2026-08-31).
  • Home office. Only if the space is your principal place of business, or you use it only for the practice and regularly see patients there. You then claim a reasonable share, such as by floor area, of heat, electricity, home insurance, maintenance, property taxes and mortgage interest, and the claim cannot create or increase a loss (CRA, Business-use-of-home expenses, 2026-08-31). Many physicians who work in a clinic or hospital do not meet these tests, so we check before claiming.
  • Vehicle. The costs of a vehicle you use to earn practice income, in proportion to business kilometres (CRA, Motor vehicle expenses, 2026-08-31). Keep a logbook of each business trip (date, destination, purpose, kilometres) for the whole year, or one full base year and a three-month sample in later years (CRA, Motor vehicle records, 2026-08-31).
  • CCA on equipment. Medical instruments costing less than $500 go in Class 12 (100%), furniture and equipment of $500 or more in Class 8 (20%), and computers in Class 50 (55%) (CRA, Classes of depreciable property, 2026-08-31).
  • The MPC salary or dividend choice. Not a deduction, but the yearly decision that changes your tax the most. Salary is earned income: it builds RRSP room of 18% of the previous year’s earned income up to the yearly limit (CRA, Guide T4040, 2025) and CPP contributions are paid on it (Canada.ca, CPP contributions). Dividends build no RRSP room and carry no CPP.

We go through this list with you before your corporation’s year-end and again for your personal return. Book a free consultation.

How do you find a CPA for a medical practice in Alberta?

Look for a CPA firm that already works with physicians who have a CPSA professional corporation, gives you a fixed fee in writing, and handles both your corporation’s year-end and T2 and your own return. What to check:

  • A licensed CPA firm. Look the firm up on CPA Alberta’s public register (CPA Alberta, Verify a member or firm). We are licensed by CPA Alberta, and every engagement is led by a Chartered Professional Accountant.
  • CPSA professional corporation experience. CPSA issues the professional corporation permit, only physicians with an active CPSA permit can hold one, the corporation’s annual renewal is separate from your practice permit renewal, and your spouse, common-law partner and children may be non-voting shareholders (CPSA, Incorporate my practice, checked 27 October 2026). Ask the firm how it handles the share structure and dividends to family shareholders.
  • A fixed fee in writing. Ask what it includes and what costs extra. Our Medical Professional Corporation and Clinic Accounting packages are a fixed monthly fee, see our pricing page, and we confirm yours in writing before any work begins.
  • The MPC year-end and the T2. Your corporation files its T2 within six months of the end of each tax year (CRA, When to file, page dated 2026-10-19), and an Alberta AT1 in the same six months unless it is exempt (Alberta, Corporate income tax). Ask whether one team prepares the year-end statements, the T2 and AT1, your T4 or T5 slips and your own return.
  • How you will work together. We serve physicians across Alberta, and in every province and territory except Saskatchewan, by Zoom, phone and email from our Calgary office.
